Congratulations on the birth of your new child and thank you for choosing our practice for their medical care.

If you have not already made your follow up appointment, please contact our office at 310-854-0770. Your child should be seen in our office when they are 2 weeks old.

Please complete the enclosed paperwork and bring it with you to your first appointment along with your current insurance ID card. Please also remember to contact your insurance carrier to notify them of the arrival of your new baby. This is very important, as claims have already been sent in for our in-patient visits.
